Individualized Housing Options
The Individualized Housing Options (IHO) process will help a person discover housing options, plan a move, learn about help and supports and develop a person-centered housing support plan that is workable into the future. Services and supports will help individuals live independently in the housing option and community of their choice.
Following the Individualized Housing Options philosophy no matter where an individual lives, help and supports can be matched to that person’s unique needs. These supports will vary based on a person’s needs. An individualized plan offers increased choice and an opportunity for individuals to self-direct the help and supports they need and desire.
Housing with Services
Housing with Services provides services to individuals who qualify for waivered services. Customized community living must be a reasonable expectation for individuals in this program. The goal of this service is to help individuals live independently in the community while managing their needs. Individuals of Housing with Services utilize the skills they have and receive support in areas of need in order that they might live in this community setting rather than a more restrictive setting.
Program staff meet with participants to work on community living skills in the following areas:
- Home Management Skills: Individual instruction in specific daily skills.
- Community Orientation: Increased familiarity and activity within the community.
- Interpersonal Communication: Problem solving through collaborative methods.
The program is not staffed 24 hours a day. Staff travel between houses depending on the day’s events and the client’s needs. Individuals can access designated staff at any time using relevant technology.
